Insurance Certificate
Request |
Whenever a Corral is going to have an event and a proof of
insurance certificate is required, this form must be
filled in and sent to the National Office in order to
receive a certificate. |

Single Event Membership Form |
A
non-ETI member may fill out a 'Single Membership Event
Form" and participate in an ETI sponsored event. |
|
Liability Release
Waiver |
All
participants in an ETI Event are required to sign a
Liability Release Waiver. One of these forms MUST be
filled out by each participant at EVERY ride or activity. |

Incident Log Form |
To
log individual Incidents at an ETI sponsored event. |
|
Accident Report Form |
To
document and repost an accident occurring at an ETI sponsored event. |

Trail Trials Rules |
ETI
sanctions a form of competition called Trail Trials. These
are trails rides where the horse and rider are judged on
how they handle various obsticles while on a trail ride.
These are the rules which govern how a Trail Trial is
conducted. |

E.T.I. has
a number of legal forms which must be filed for various events. In
an effort to make these form more readily available to our
membership, these forms are being made available on the National
